Referral Process & Our Communication Promise
At Ark Animal Hospital's Physical Therapy & Rehab Center, we believe the best patient outcomes are achieved through collaboration. We are honored to partner with referring veterinarians to help patients improve mobility, manage pain, recover from injury or surgery, and enhance their overall quality of life.
To refer a patient, please complete the referral form below. To ensure our team can provide the most appropriate care, the following records need to be either attached to the form or emailed to physicaltherapy@arkanimalhospital.net
- Medical Records about the affected area/ concern
- Your treatment goals for your patient
- Radiographs/Imaging for the affected area, if already done
- Any surgical records for the affected area
- Labwork going back 1 year
Our Communication Promise
We recognize that you are your patient's primary veterinarian, and we are committed to maintaining open communication throughout the rehabilitation process.
When you refer a patient to our Physical Therapy & Rehab Center, you can expect:
- A comprehensive consultation report following the initial evaluation
- Timely updates regarding significant findings, treatment recommendations, and changes in the patient's condition
- Progress reports at key milestones throughout the rehabilitation program
- Prompt communication if additional diagnostics, medical intervention, or reassessment by your team is recommended
- Ongoing collaboration with your hospital to support the best possible outcome for our shared patient
